On Monday September 20, Health Hero (HNH Immunizations) will be at all Gadsden City Schools to administer the flu
vaccine. This vaccine is OPTIONAL and is being offered to your student at no cost to you. If you have insurance, your
insurance company will be billed, but you will not be charged. There is no charge if your student is uninsured. Due to a
contract with Worksite Wellness, students with PEEHIP insurance are unable to participate in this clinic.

If you would like your student to participate in this clinic, please fill out and sign the consent form available at your child’s school.

Only return the completed consent form if you want your student to receive a flu vaccine.

Any student that refuses, even with a signed consent, will not be vaccinated.
